This Office of State Fire Marshal (OSFM) will make every effort to accommodate your Public Records Requests (PRR).

Please use the attached form to submit your Public Records Request.

This form may be completed online, saved to your PC and submitted via E-mail as an attachment or printed and submitted via regular mail. To submit your request by E-mail, please send it to [email protected].
